Creating Form Letters for Signature

Form letters that are available to send out for Signature are created the same way as other form letters in AppColl and uploaded to the FormLetters folder. For more details on the basics of setting up a form letter click here.

To insert recipient information into the form, and to indicate where the recipient should sign, you should use form fields similar to {Signature.Recipient…}. These form fields work in a manner similar to {Matter.Inventor…}, which allow access to contact information associated with inventors for a matter. Any information in the recipients contact record can be accessed using {Signature.Recipient…}.

 

Documents with a Single Signer

For signature documents associated with only one signer, you can use the following form fields:

Form Field Description
{Signature.Recipient.SignatureLocation} Special field - Indicates where the recipient should sign
{Signature.Recipient.SignatureDate}  Special field - Indicates where the where the signing date will automatically be added
{Signature.Recipient.<contact field>} Contact field - e.g. Email, Initials, Name, First, Address

Important: If you use one of the form fields above for a document with a single signer, AppColl will automatically duplicate the document for every recipient of the envelope. AppColl will prepend the name of the recipient to the document name when added to the envelope.

 

Documents with Multiple Signers

For situations where more than one recipient is signing a single document (meaning more than one recipient is specified and the “Send to all recipients as one transaction/envelope” checkbox is checked), use form fields similar to {Signature.Recipient<n>…}, where n is the order of the recipient as specified in the Signature Rule or in the Add Signature pop-up. For example, with the following information specified:

 

 

To insert information for Jimmy Page in the documents, use {Signature.Recipient1.xxx}, for Eddie Van Halen, use {Signature.Recipient2.xxx} and so on. If you specify Matter.Inventors as the Recipient, then the order of recipients is as specified in the matter.

If the document has multiple signers, you can specify, using the “Sign Sequentially…” checkbox, that the recipients sign in the order listed or they all get the document to sign at the same time.

Important: For a form letter to be available for use as a signature document and appear in the list it must contain a {Signature.Recipient…} form field.

Note: the special form fields that were used previously will still work for documents with one recipient, but are now deprecated.

Note that in signature document forms all {Contact…} form fields refer to the recipient, or signer, of the document. 

 

Signature Form Example (Single Signature)

SAMPLE PATENT RIGHTS ASSIGNMENT

{Signature.Recipient.Name(ToTitleCase)} ("Assignor") is owner of “{Matter.Title}”, as described in the U.S. Patent Application with U.S. Patent and Trademark office Serial Number: {Matter.ApplicationNum}, filed {Matter.FilingDate(MMMM d, yyyy)}, (the "Patent Application"). {Matter.Client} ("Assignee") desires to acquire all rights in and to the Patent Application and the patent (and any reissues or extensions) that may be granted.

Therefore, for valuable consideration, the receipt of which is acknowledged, Assignor assigns to Assignee 100% of his or her right, title, and interest in the invention and Patent Application (as well as such rights in any divisions, continuations in whole or part, or substitute applications) to Assignee for the entire term of the issued Patent and any reissues or extensions that may be granted and for the entire terms of any and all foreign patents that may issue from foreign applications (as well as divisions, continuations in whole or part, or substitute applications) filed claiming the benefit of the Patent Application.

Assignor authorizes the United States Patent and Trademark Office to issue any Patents resulting from the Patent Application to Assignee according to the percentage interest indicated in this assignment. The right, title, and interest are to be held and enjoyed by Assignee and Assignee's successors and assigns as fully and exclusively as it would have been held and enjoyed by Assignor had this assignment not been made.

Assignor further agrees to: (a) cooperate with Assignee in the prosecution of the Application and foreign counterparts; (b) execute, verify, acknowledge, and deliver all such further papers, including patent applications and instruments of transfer; and (c) perform such other acts as Assignee lawfully may request to obtain or maintain the Patent for the invention in any and all countries.
